Abstract: 本研究は,精神看護学実習における学生の学びを把握することで,教員の指導方法について検討し,今後の実習指導の課題を明らかにすることを目的とした.対象は,本学看護学科3年生46名,前期の精神看護学実習終了後の「精神看護学実習で学んだこと」の記録をデータとして学びの内容分析を行った.その結果,22のサブカテゴリーと,10のカテゴリーが抽出された.そのうち「精神医療・治療の実際」「他者理解に向けた手段」「自己理解・自己洞察」「看護観・障害者観」「看護の役割の実際」「自己と他者との関係及び,相互作用」のカテゴリーはいずれも50%以上の記述があった.最も記述件数が低かったのは,「患者の権利と人権」「疾患・症状の理解」であり,次いで「ソーシャルサポート」「看護の展開」であった.また,教員が期待する対象の生活背景,処遇に関する患者の心理面の記述はなかった.これらの内容分析から,(1)自分の感情を表出し自己を受容できる(2)実習に対する不安や精神障害者に対する偏見を軽減する(3)生活者として患者を捉えられる(4)地域精神医療,社会復帰支援の学びを深めるなどの,指導目標と指導課題が示唆された.
The purpose of this study is to explore teaching methods and clarify the issues of how to guide students in nursing practice settings in the future by comprehending how and what students learned from the psychiatric nursing practices. Forty-six senior students of this department were subjected to this study. The subjects were asked answer the questionnaire of "What you have acquired from the psychiatric nursing practices" and the analysis was conducted based on the questionnaire. The results were sorted into 22 subcategories and 10 categories. The students answered 50% either of following categories, [Psychiatric treatment ? reality of treatment], [Procedure for how to understand others], [Self understanding?self insight], [Concept of nursing ? concept of handicaps], [Reality of nursing role], [Relation and interaction between oneself and others]. The categories for which the students least answered were [Patient's right, human right] and [Understanding of disorder and symptoms], followed by [Social support] and [Dissemination of nursing].However, the answers for the subjects' backgrounds and how patients perceive the way they are treated could not be obtained, which teachers expected to know. The outcomes of this analysis suggest following teaching objectives and assignments to facilitate students' learning process; (1) To guide students to acquire skills to express feeling and accept themselves, (2)T o minimize anxiety against nursing practices and prejudice against psychiatric patients, (3) To guide student to acquire skills to regard patients as livers, and (4) To expand the learning activities in the community psychiatric treatment center and social rehabilitation support settings.
